1- On the form below, indicate your name and contact information, then the name of the person to whom you want to offer a one-year membership in the Demers Families Association, indicate their email address and/or phone number and click Send.
2-Pay the membership according to the instructions provided and indicate it is a gift.
3- Once the payment is received, our Secretary will contact the recipient to inform them of your gift and ask them to fill out the rest of the usual New Member Form.
